Privacy statement for Nurture Paediatrics
At Nurture Paediatrics, we are committed to protecting your privacy and the confidentiality of your personal and health information. By consenting to our Privacy Policy, you agree to the terms outlined below, which comply with the Privacy Act 2020 and the Health Information Privacy Code 2020.
Collection of Personal and Health Information
We will collect personal and health information directly from you unless you have consented to us collecting the information from someone else or another exception applies.
Information is collected solely for the purpose of providing you with the best possible care and for any legal purposes.
You will be informed about why we are collecting your information, who will have access to it, and your right to access and correct it.
We may collect information from children and young people in a fair manner.
Use of Personal and Health Information
Personal information collected for one purpose will not be used for another without your consent unless an exception applies, such as publicly available information or usage in a way that does not identify you.
We ensure that the information we use is accurate and up to date.
Storage and Security of Information
Personal and health information will be stored securely to prevent unauthorized access or use.
We retain health information for as long as needed to treat you and for a minimum of 10 years from the last treatment date.
Information will be destroyed in a manner that ensures confidentiality, such as shredding documents.
Access and Correction
You are entitled to ask if we hold any information about you and to access this information unless we have lawful reasons to withhold it.
You can request that any information we hold about you be corrected.
We will assist you in accessing or correcting your information.
Disclosure of Information
We will not disclose your information without your consent unless:
It is for the purpose of your treatment (e.g., a referral).
It is to your caregiver, and you have not objected.
It is necessary to prevent a serious and immediate threat to your or another person’s life or health.
It is for the purposes of a criminal proceeding.
It concerns suspected child abuse and is disclosed to a social worker or the police.
Privacy Breaches
In the event of a privacy breach, we will notify affected individuals and, if the breach poses a risk of serious harm, the Privacy Commissioner.
Serious harm includes physical harm, financial fraud, family violence, and psychological or emotional harm.
Confidentiality and IT Security
All staff have signed a confidentiality agreement extending beyond their employment period.
Confidential materials are either shredded on-site or placed in secure destruction bins.
Each staff member has a unique login protected by a strong password, with automatic reminders to change passwords every six months.
